All long-term care and Assisted living applications begin with an eligibility assessment process conducted by a case manager from your local health authority home and community care team.

Step 2
Confirmation of approval for a funded spot and placement on a waitlist.
Step 3
Book a tour with us if we are your preferred care home.*
Step 4
Receive a call to move in. The call is determined by your spot on the waitlist.**
* Tabor does not schedule tours until steps 1 and 2 are completed. ** Bed availability and waitlist spots are determined by your local health authority home and community care team and all questions on waitlist status must be directed to them.
